The partnership aims to give patients at more than 35,000 PatientPoint locations the opportunity to connect with and receive support from others living with the same chronic health issues.
Screens in PatientPoint’s waiting rooms and exam rooms display messages promoting MyHealthTeam’s platform, which serves patients through a growing network of more than 60 patient communities.
When patients scan a QR code with their PatientPoint device, they become part of a relevant online community where they can receive support from other patients and information to guide them in managing their care.
The companies, who participated in a five-week trial program, say the partnership has seen strong engagement among patients with obesity and diabetes.
